If you recently added Donations Tracker you can toggle the showing of the Donate tab like from the Application Settings page under "Account" at the top right of the page:

Facebook changes
Facebook recently removed the option for new application users to add the profile box, from now on people adding applications can only add the profile "Tab" which appears at the top of your profile:
